HEALTHY COMMUNITIES

The Plett Wellness Programme aims to promote health and wellness and prevent illness – building healthy, compassionate and responsible communities – through 5 interlinked programmes:

  • A mobile health screening, testing and referral service, assisting businesses with employee wellness;
  • A door-to-door household assessment, health screening, education and referral service;
  • Community outreach service to assist Primary Health Care (PHC) clinics to reach patients who can not access health care services due to illness or disability;
  • Partnering with all health and welfare service providers in each area to maximise the reach of these service and collectively improve health outcomes in the region.
  • Counsellors in PHC facilities assisting with HIV and TB screening, and medication adherence counselling.
